ABOUT THE PROGRAM

One of the hottest topics in health care today is the avoidance of hospitalizations for Medicare beneficiaries. At the same time the incidence of Congestive Heart Failure (CHF) is dramatically increasing. These two facts seem to spell doom as the baby boomers enter retirement years and become Medicare beneficiaries. With the impact of CHF related health conditions, hospitalizations and re-admissions, it will be imperative for long-term care facilities, hospitals and their partners to minimize re-admissions through greater knowledge and understanding which will create better resident outcomes through physicians, physician extenders, staff, resident, and family education.
